Yes, all medications tend to lose efficacy with time. The elements, such as heat, light and moisture cause them to degrade. Thus, I have to say that after 15 years, yes, you should just dispose of it and obtain a new supply, if you need it.
The FDA lists the typical side effects of Quinine as possibly including nausea, diarrhea, dizziness and stomach pain.
